Family 1st Program

The Family 1st Program is a collaborative partnership between First 5 Santa Clara County and Roots Community Health. The program supports families with children under six and is designed to foster healthy development, strengthen family bonds, and provide resources to meet each family’s unique needs. 

What We Offer: 

  • Each family is paired with a Health Navigator, who provides personalized assistance with various needs relating to the child and family

  • We use Ages and Stages Questionnaires (ASQs) to assess childhood development and ensure children are on track for school readiness

  • Families gain access to essential tools and resources to promote healthy living and nutrition 

  • Families connect through peer support groups and parenting classes, fostering a sense of encouragement and shared growth

Doula Program

Roots Community Health offers free sister friend/ doula support services for all pregnant and postpartum women of African Ancestry in Santa Clara County.

The Doula program offers: 

  • Personal support during pregnancy, childbirth, and postpartum

  • Assistance with connecting to local resources 

  • Access to virtual and in person events that focus on wellness, nutrition, and fitness 

Prenatal Support includes: (but is not limited to) 

  • Child birthing classes that address: 

    • Preparation for the birthing experience for you and your support partner

    • Signs and stages of labor

    • Pain management techniques

    • Birthing exercises 

  • Assistance with developing and implementing personal birthing plans 

  • Lactation/ breastfeeding education 

  • Emotional support throughout the pregnancy and birthing experience 

Postpartum support includes: (but is not limited to):

  • Assistance with developing and implementing a postpartum plan
  • Education on newborn care
    • Swaddling
    • Infant nutrition
    • Safe sleep practices
    • Recognizing infant cues
  • Lactation/ breastfeeding support
  • Nutrition education and meal assistance
  • Emotional/ mental wellness support throughout the postpartum period

Roots doulas will not perform clinical evaluations (including but not limited to vaginal exams, fetal heart tones, blood pressure checks, etc.)

Women of Wellness Program

The Women of Wellness (WoW) Program is dedicated to empowering women of reproductive age by addressing three key pillars of health: physical health, nutritional wellness, and mental well-being. This comprehensive program is designed to combat the challenges of poor health outcomes and provides participants with access to trusted professionals, valuable resources, and personalized maternal health navigation.

Our goal is to create lasting change by supporting the whole health of each individual. Through a holistic approach, the WoW Program works to improve maternal health, enhance well-being during and beyond pregnancy, and achieve better birth outcomes for both mother and baby.
