The social determinants of health that apply to individuals are applicable to families, neighborhoods and communities. They also evolve with the needs of those we serve. For those reasons, our work extends beyond the services of a traditional clinic to include housing and food security, educational attainment, workforce development, advocacy, and community capacity building. This approach is critical to uplifting communities impacted by systemic inequities and poverty.

Roots offers a wide array of programs and services dedicated to healing and supporting individuals to move forward from homelessness or transition from incarceration.
Unemployment or underemployment can be barriers to overall health and wellness. Roots provides a diversity of training and operates workforce enterprises dedicated to helping our members acquire employable skills and achieve economic stability.
Individual health is linked to the overall wellness of the communities in which we live. In partnership with residents, businesses and organizations, we lift up our most pressing issues, identify policies and systems that must change, and advocate at the local, state and national level to enact those changes.
