I-693 Examinations: What You Need To Know

I-693 examinations are also known as the “Report of Medical Examination and Vaccination Record”. The I-693 examinations are conducted by a medical doctor and are part of the paperwork package necessary to establish permanent residency. An I-693 is required to insure that an applicant is not inadmissible to the United States on the basis of public health grounds.

The I-693 consists of a physical examination and a review of records. Applicants over the age of 15 are required to have (or present proof of) a syphillis test and applicants over the age of 2 are required to have (or present proof of) a TB test. Other blood tests might be required, as well. The applicant also needs to have (or present proof of) vaccinations or immunity (via blood titer) for tetanus-diphtheria (within the last 5 years), measles, mumps, rubella and varicella (chicken pox).

The patient fills out Part 1 of the I-693 Form (without signing or dating it) and the physician fills out the rest and certifies the form. The I-693 is presented to the INS or the attorney in a sealed envelope, and the sealed envelope becomes part of the INS application package. Applicants are instructed to not open the envelope, as this invalidates the I-693 examination.

Roots Community Health provides these examinations at two community locations. Exams are by appointment only. Our fee structure consists of a base cost for the physical exam, vaccine verification, the completed, certified and sealed I-693 form, and a copy of the form for the applicant.
